Vaccination Clinic for Students & Families
Northshore School District is partnering with Safeway/Albertsons for a free vaccination clinic for students ages 12+ and their families.
If you are receiving your second dose, you will need to bring your vaccination card to the June 12 clinic. Students and families may also sign up to receive the 1st dose of the Pfizer vaccine on June 12.

Have your first Moderna vaccine dose?
If you have already received your first dose of the Moderna vaccine, you can walk in to any of the clinics and receive your second dose. You must have your vaccination card.
What to Expect
- Please bring with you and have ready at check-in:
- Your Northshore School District Identification and/or student ID number. If your student is not registered in our system, your eligibility to be vaccinated will depend on the available supply.
- Your health insurance card. However, you will NOT be turned away if you do not have health insurance.
- When you arrive, we will take your temperature and ask you questions about COVID-19 symptoms, exposures, and travel.
- You cannot receive a dose of the vaccine if you have been vaccinated with anything between May 30, 2021, and June 12, 2021 (two weeks prior to this shot).
- If you have access to a printer, please complete and print the Informed Consent and Release of Information form and bring it with you to the clinic. We will also have copies of the form available for you to complete at the clinic. Parent/guardian signature is required if the student is under the age of 18. Parents may accompany their student(s) to their vaccination appointment.
